  --- Experience: Minimum 3 years in acute inpatient care, Med-Surg, and stroke patient management.
  --- Education: BSN required (CCM or related certification preferred).
  --- Skills: Strong understanding of medical necessity, LOS management, care coordination, and resource utilization.
  --- Software: Experience with EPIC preferred.
  --- Conduct comprehensive patient assessments and care planning.
  --- Utilize InterQual screening to determine medical necessity.
  --- Coordinate care for 18-22 patients, ensuring cost-effective and quality outcomes.
  --- Collaborate with healthcare teams to optimize LOS and resource utilization.
  --- Manage insurance eligibility, benefits, and contract requirements.
  Ideal candidates will have 5+ years of experience in Utilization Management (UM) and Case Management (CM).
