Job Description:
POSITION SUMMARY: The Case Management Program Liaison provides
and coordinates a full array of outreach-based services for clients
in the DMHAS/ERSC program. The position may also include some
programmatic and administrative support to the ERSC program. This
position will represent ABH and the ERSC program in a positive and
professional manner and adhere to ABH's best practices, guidelines,
policies and procedures as established, promoting ABH as a leader
in behavioral healthcare in all interactions and work. This
position is stationed in Norwich DUTIES AND RESPONSIBILITIES:
- Provide telephone support for ERSC referral calls - ensuring
prompt voice response to consumers or providers requesting case
management services;
- Provide full array of Case Management services, including
services that assist clients in obtaining basic supports needed to
improve their likelihood of success in recovery. Including, but not
limited to, the following:
- Outreach and engagement attempts with clients in various
community settings, including clients' home, shelters, soup
kitchens. Outreach efforts may include attempts to contact clients
via visits and/or letters t last known address, phone calls, visits
to shelters, soup kitchens,
- Service planning for non-clinical needs (i.e. housing);
- Assistance with arranging transportation to treatment and other
community supports appointment;
- Transportation t essential appointments when no other
arrangement is timely or/and practical and such transport can be
safely achieved;
- Assessment and referral to the appropriate level of care;
- Arrange and attend case coordination meetings with clients and
treatment providers to assure that appropriate issues are being
addressed in treatment
- Service planning for non-clinical needs
- Assistance with arranging transportation to treatment and other
community supports appointments;
- Transportation to essential appointments when no other
arrangement is timely or/and practical and such transport can be
safely achieved using own personal vehicle;
- Assistance and advocacy in accessing extant community support
services;
- Face to face supportive counseling;
- Provide transportation to clients, using personal vehicle
- Provide coordination and monitoring of staff when ERSC Regional
Coordinator is not available.
- Monthly Chart reviews;
- Attend all required staff, supervision and training meetings
(held throughout Connecticut);
- Produce reports as required within established time frames;
monthly, quarterly and annual reports such as the Balanced Score
Cards, Performance Measures, Agency specific reports, and referral
trending
- Coordinate staff schedules for coverage
- Assist with preparation of the agenda and complete minutes for
case management staff and advisory council meetings;
- Monitor Willimantic Outreach Case Manager data and submit
reports to Regional Coordinator.
- Perform duties and assignments related to supporting the ERSC
Program and maintain familiarity with and understanding of
protocols;
- Maintains confidentiality of all client protected health
information and adheres to all HIPPA related policies and
procedures and abides by all State and Federal Confidentiality
Regulations;
- Demonstrates ethical behavior and cultural sensitivity in all
activities involving individuals of diverse backgrounds.
- Comply and work towards goals established by DMHAS and ABH
- Carry a cell phone during work hours (8 a.m. - 5 p.m.) and
respond to any messages within 30 minutes of the message;
- Performs other tasks/responsibilities as required to support
the business operations; EDUCATION AND EXPERIENCE REQUIREMENTS:
- Associate's Degree in Human Services or related field
preferred
- At least two years experience working with individuals who have
substance abuse or co-occurring diosorders.
- Attend annual Conflict of Interest training.
KNOWLEDGE/SKILLS/ABILITIES:
- Strong attention to detail; ability to work on multiple tasks
and meet deadlines;
- Excellent PC skills with demonstrated experience using
Microsoft Office Package (MS Word, Excel, Power Point, Access,
Outlook); internet;
- Excellent data entry skills - input date into various software
packages;
- Clear and concise written and verbal communications skills
required;
- Outstanding customer service skills;
- Excellent organizational skills;
- Flexibility and ability to manage multiple tasks is
essential;
- Valid Connecticut Drivers License; good driving history and
reliable transportation;
