Job Description:
ESSENTIAL FUNCTIONSAssist with patient care including the
following:Assist with patient flow and daily scheduleMedication
reconciliation of complex medication listsPerform point of care
testing as directed by providerGive injections and immunizations as
directed by providerTake vital signs and weightExecute protocols
and standing orders as appropriate to scope of practicePerform
point of care testing; oversight of testingAssess patient adherence
to prescribed medications, dosing plan and identify barriers to
adherence and provide a solution driven approach to minimize
barriersAssess patients for compliance to medication regimen,
signs/symptoms of bleeding or clotting, mechanism of drug action,
dose, tablet strength, goal of therapy, route of administration,
interacting food and drugs, importance of regular dental care,
importance of complianceContinually evaluate and document patient's
response to anticoagulation goals and protocols in the medical
recordRefill patient medications according to protocol per provider
approval and oversightProvide follow up with patient after hospital
and /or skilled facility dischargeCoordinate patient care
management with in house specialties and resources; community
resources; home health and other health care agencies; hospital
careManage telephone encounters utilizing triage and telehealth
principlesDocument all clinical data correctly and appropriately in
patient's EHRPrepare, stock and clean exam room(s), maintain
supplies and equipment for treatment, including routine checks,
calibration, cleaning and repairingOversight of clinical
environment with attention to safety and infection controlMaintain
confidentiality and compliance in all patient care
activitiesSpecial projects as assigned by lead, coordinator or
managerJOB REQUIREMENTS Registered Nurse; graduate from an
accredited school of nursingUnencumbered active registered nurse
licensure in state of MaineProfessional appearance and
demeanorProficient in Windows applications, electronic medical
recordAbility and willingness to provide excellent customer service
in person and over the telephone to patients while utilizing
professional judgment, triaging, phone skills, and decision making
abilityAbility to prioritize and handle multiple tasksWork
collaboratively with all team membersDemonstrate critical and
reflective thinking
