Social Worker - Coordination of Care

- This exciting new social worker position is designed to address
the primary behavioral health treatment needs for patients
discharged from the emergency department. It involves improving the
coordination of care services amongst the emergency dept,
psychiatry, and primary care in order to enhance population health
for our children and adolescents in the state of Maine. Primary
duties focus on engaging in brief targeted treatment with patients
to serve as a "bridge" until appropriate services are secured,
facilitating skills groups, and working with multidisciplinary
teams on 2 progressive and innovative behavioral healthcare grants
awarded to MMC.
This position is a 2 year, grant funded position.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: Knowledge of social work theories, techniques and
therapies, as used in individual, family and group treatment, as
well as principles of social welfare as acquired through completion
of a Master's degree in social work from an institution accredited
by the Council on Social Work Education.
Experience: Two or more years work experience in a health or mental
health setting preferred.
License/Certification: Licensure by the Maine State Board of Social
Work Licensure as an LCSW or LMSW-CC.
Ability to communicate effectively and work collaboratively with
all members of the multidisciplinary healthcare team.
