Position Summary
At Trellis Health, we are on a mission to transform primary care.
We seek a Physical Therapist who is not just skilled in their role,
but is passionate about pioneering innovative healthcare solutions.
As part of a start-up subsidiary of Maine Health, you will play a
crucial role in shaping our clinical practices and patient
experiences from the ground up. This position requires flexibility,
a high level of autonomy, and a proactive approach to both
problem-solving and patient care.
The PT leads the Trellis Health clinic's MSK program and is
responsible for assessing, diagnosing, and treating patients with
musculoskeletal issues. This pivotal role involves not only
providing direct therapy but also advising the care team on
MSK-related care plans and integrating physical therapy into
comprehensive patient care strategies. The PT coordinates with
other Trellis Health team members to optimize ambulatory management
while collaborating with the care team on diagnosis and management
decisions, including appropriate use of imaging and specialty
resources. The PT's expertise is integral to the multidisciplinary
team, contributing to a cohesive approach that enhances patient
functionality, reduces pain, and optimizes patient experience and
overall health outcomes in a team-oriented care setting.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: Graduate of an APTA accredited Physical Therapist
program required.
License/Certifications: Current applicable state(s)
temporary/permanent license as a Physical Therapist required or in
compliance with the state practice act. Current BLS certification
required or must obtain within 30 days of start date. Positions in
Home Care will require current valid Driver's License with vehicle
available for work covered by liability insurance as specified by
agency policy.
Experience: Three years of experience working in a hospital,
long-term care, home, school system, or outpatient Physical Therapy
department preferred
Knowledge of division policies and procedures, advanced skills used
in operating equipment and materials used in conducting physical
therapy treatments, as well as proficiency in performing such
treatments.
Analytical ability necessary to conduct a thorough evaluation of
patient abilities and disabilities, physical strength and
limitations, develop sound treatment plans, and assess patient's
progress.
Strong computer skills with skill in using health information
management systems and data.
Excellent communication, interpersonal and customer service skills
with the ability to establish/maintain effective working
relationships with patients, clinical staff and the public.
Ability to demonstrate compassion and desire to help patients.
Skills necessary to participate as a member of an interdisciplinary
healthcare team.
Ability to maintain patient confidentiality.
