This role involves facilitating communication between patients and the interdisciplinary team while utilizing technology to document nursing services. Candidates must possess a valid CT LPN license and have at least one year of experience, with homecare experience preferred. Qualifications

Licensed Practical Nurse with Valid CT LPN license One year of LPN experience; Homecare experience preferred. Valid driver's license and reliable transportation required. Computer proficiency required.
