Now Hiring: RN Care Manager – York, Maine Position Details: Start
Date: October 31, 2025 Contract Length: 13 weeks Hours: 40
hours/week Schedule options: 8:30 AM–4:30 PM OR four 10-hour shifts
(7:30 AM–5:00 PM) No on-call required Weekend & holiday rotation
required Responsibilities: Conduct patient assessments and
coordinate discharge planning Collaborate with interdisciplinary
teams to ensure high-quality care Manage utilization review and
ensure smooth care transitions Communicate effectively with
patients, families, and care partners Qualifications: Active Maine
RN license (must be current) 3–5 years of clinical experience
(Med/Surg, Case Management, or Home Care) Utilization review and
discharge planning experience required BLS certification required
