(40 hours per week) Hours: 8:00 AM - 4:30 PM (flexible) Salary:
$55,000/annually Job Description Develops and maintains
professional relationships with staff, physicians, residents,
families, community resources, and third-party payers. Completes
Admission forms/assessments and documentation as outlined in the
Social Service Admission Process. Completes a plan of care for each
resident based on their needs. Documents on progress towards care
plan goal, and updates POC with significant change and/or
quarterly. Assess residents various health needs on an on-going
basis. Assess the need for referrals to health services and
provides education and support. Qualifications Experience in
Long-Term Care or medical setting preferred Comfort and experience
working with dementia populations a plus Flexibility to assist in
our skilled, assisted, and residential living areas A four-year
undergraduate degree in social work, psychology, or a related
field.
