Management Center provides comprehensive and continuous quality

outcomes to state and national benchmarks. Advanced Practitioner
Role Scope The APP will be dividing their time between SNF setting.
The APP will play a vital role in filling the gaps and needs of our
heart failure population, educating our patients and families
lifestyle modifications, GDMT and providing an additional resource
to the affiliated cardiology practices. Shift: Full Time – M-F
8-4:30 Advanced Practitioners Enjoy At Hartford HealthCare newly

Advanced Practitioner Requirements Graduate of an accredited
Physician Assistant or Nurse Practitioner program Licensed, or
eligible to be licensed, as a Physician Assistant, or Advanced
Practice Registered Nurse (APRN) in the state of Connecticut
Certification or eligible to be certified as a Physician Assistant
or National Board Certification as an Advanced Practice Registered
Nurse Cardiology experience required Geriatric board certification
