Job Description:
Signify Health is seeking a dedicated and compassionate In-home
Traveler Nurse Practitioner (NP) or Physician Assistant (PA) to
join our innovative team. This role is pivotal in providing
top-tier at-home healthcare services as part of our value-based
payment programs, utilizing state-of-the-art technology and
analytics. The ideal candidate will deliver personalized,
patient-centric healthcare directly to patients' homes, managing a
wide range of medical conditions through comprehensive assessment,
diagnosis, and treatment planning in coordination with patient care
teams. This position requires extensive travel within the assigned
region and flexibility in scheduling. Conduct in-home health
assessments including physical examinations, evaluations, and
patient interviews. Develop and implement treatment plans based on
patient assessments and health data. Collaborate with patient care
teams to ensure continuity of care. Document and maintain patient
records with high accuracy and in compliance with all legal and
regulatory requirements. Educate patients and their families on
treatment and care plans, as well as preventative health measures.
Adjust care plans based on patient progress and updated health
information. Travel extensively within the designated area to
provide in-home patient care. Proficient in using medical software
for patient data and electronic health records. Strong diagnostic
skills and proficiency in developing detailed treatment plans.
Excellent interpersonal and communication skills. Ability to work
independently and manage time effectively. Strong ethical standards
and the ability to maintain confidentiality. Medical Insurance
Dental Insurance Vision Insurance 401k Current licensing as a Nurse
Practitioner or Physician Assistant. Board Certification in Family,
Adult, or Geriatric Care (as applicable). Valid driver’s license
and reliable transportation, necessary for regional travel.
