treatment options their patients require, they turn to us. The
Community Care Clinic (CCC) is located at 132 Jefferson Street, at
Hartford Hospital. CCC clinic sees around 4,000 patient visits
annually with an average of 30-40 patients per day. The Division of
Infectious Diseases provides inpatient and outpatient consultation
regarding the diagnosis and management of all types of infectious
diseases. Our staff includes Attending Physicians, Fellows,
Psychiatry, Psych Residents, Social Worker, Nutritionist, Pharmacy
Liaison, APRNs, RNs, MA/MAAs, Case Managers and Program Manager who
provide compassionate care to a diverse population of patients. CCC
is a Ryan White funded program. A majority of our patients are
bilingual, with Spanish being their primary language, and have
healthcare coverage under Medicaid. Our specialists are skilled at
treating many infectious conditions, including HIV infection,
Hepatitis, fever of unknown origin, recurrent infections or rashes
of unknown type or origin, Influenza, opportunistic infections in
patients who are immunosuppressed due to acquired or congenital
immunodeficiency, transplant or other medical condition. CCC guides
patients through the health system, including appropriate referrals
for services to other health professionals. CCC also has additional
programs such as Addiction Medicine and a Tobacco Cessation
Program, as well as a Travel Clinic. Job Summary: The Tobacco
Treatment Specialist provides behavioral counseling to help current
smokers and/or vapers quit. Working under the direction of the
Manager of Tobacco Cessation of the STOP Smoking & Vaping
Cessation, the specialist will be responsible for assessing and
counseling outpatients who smoke and/or vape. Job Responsibilities:
• Conducts new and follow up patient assessment sessions via
telehealth or in person. Assists patients in moving through the
stages of change by providing supportive counseling in a series of
one-on-one coaching sessions or group sessions. Manages relapse
prevention, follow up and after-care services. • Completes
assessments via in person or telehealth. • Provides support for
smoking/vaping counseling as needed. • Provides resource
information and motivational coaching based on patient’s completed
assessment and stage of change for smoking cessation. • Conducts
carbon monoxide and spirometry testing as needed. • Develops
individualized treatment plans, including appropriate medications
for patients in collaboration with the patient’s medical team.
Documents the counseling sessions along with the plan of care for
each patient. • Schedules outpatient counseling and follow up
sessions. • Assists Manager of Tobacco Cessation to maintain
patient database on smoking cessation. • Assists Manager of Tobacco
Cessation to track smoking quit rates for outpatients. • Assists
Manager of Tobacco Cessation to run reports on the patient volume
and documentation compliance. • Perform other related duties as
required Qualifications • Bachelor’s Degree in health education,
psychology, social work, nursing, respiratory therapy or related
field. • 2 years of direct patient care experience with chronic
diseases and consequences of smoking. • Experience in counseling
and/or group facilitation is highly desirable; 6 months experience
in tobacco treatment is highly desirable preferred. • Tobacco
Treatment Specialist certification is required within 6 months of
employment. • Current Tobacco Treatment Specialist
certification,preferred. • Ability to communicate with patients of
all languages through interpreter services. • Bilingual Spanish
strongly preferred. Knowledge, Skills and Ability Requirements •
Applicants should have good clinical and patient counseling skills
as well as interest and ability to facilitate group programs. •
Proficiency in word processing and database management skills is
necessary. • Familiarity with Microsoft Access, Excel and Word
preferred. • Must demonstrate strong organizational and
communication skills and ability to work independently but also
