Position Requirements: Education: Valid State of Connecticut License as LPN or above; BSN preferred. Experience: Minimum of 2 years practical nursing experience in caring for seniors; including long-term care, sub-acute and rehab. CPR certified. Knowledge of State, Federal, OSHA, CCAC regulations and standards. ESSENTIAL FUNCTIONS: Provides professional nursing triage services and health advice to patients in person and over the phone Assesses patient symptoms and provides appropriate medical advice on treating symptoms and/or recommendations for escalating treatment Gives support and consultation or direction to the on-duty Security staff Refers Residents and Members to other community resources as needed and follows up with Residents and Members who received services Works collaboratively with department managers to identify resources and compliance needs. Communicates significant findings, problems, or changes in condition to Community Nurse Supervisor Follows established protocols and documents all calls Determines if in-person nursing care is required and provides care in Resident’s or Member’s home when necessary Tracks common health concerns and creates appropriate communications to educate Residents and Seabury At Home Members. Keeps current with medical trends, skills and regulations. Conveys a professional and caring image. Abides by residents’ rights. Understands and supports Seabury’s mission and philosophy.
