Job Description:

Encompass Health is seeking a dedicated Case Manager to join our team of professionals providing high-quality post-acute care across 42 states. This role is instrumental in facilitating patient-focused health care, managing patient treatment plans, and ensuring continuous delivery of integrated services following hospital discharge. The Case Manager will act as a liaison among patients, their families, and healthcare providers, to ensure efficient treatment and utilization of resources. This position offers the opportunity to make significant impacts on patient outcomes by optimizing the healthcare processes and advocating for patient needs. Assess new patients, reviewing their medical history and individual needs. Develop, implement, and review personalized care plans in cooperation with healthcare professionals. Coordinate and attend patient care meetings with the medical team. Act as the primary point of communication for patients, families, and healthcare providers. Monitor and evaluate patient progress and modify care plans as necessary. Advocate for patients’ needs and coordinate services to ensure timely medical treatment. Assist with the discharge planning and educate patients and families about the post-discharge care and resources. Strong interpersonal and communication skills Ability to manage multiple cases and priorities simultaneously Proficiency in electronic health record systems Strong problem-solving abilities and critical thinking skills Knowledge of healthcare laws, regulations, and standards Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred Active Registered Nurse (RN) license in the state of practice
