Hospital’s 4A Transitional Care and Discharge Unit is a 12-bed unit
that will be opening permanently to reduce Emergency Department
holding and to promote better throughput. Patients in this unit
consist of those who are transitioning from acute care to home or
another care setting and patients who no longer require the
intensity of acute hospital care but still need specialized
medical, nursing or other care before discharge. As an RN on the
team, you will deliver exceptional care throughout the patient care
journey. In this role, you will interact directly with patients and
their families, offering the opportunity to enhance their
experience, provide healthcare education as needed, and ensure that
they are equipped to leave the hospital care setting. We realize

need, no matter what life brings. A PRN caregiver in this position
works varying days from 7:00AM – 7:30PM. A caregiver who excels in
this role will: Provide direct nursing care. Implement and monitor
patient care plans. Monitor, record and communicate patients’
conditions. Administer prescribed medications and treatments. Note
and carry out physician and nursing orders. Assess and coordinate
patients’ discharge needs. Educate patients and their families.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of professional nursing (ADN or
BSN program) Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) certification through the American Heart
Association (AHA) or American Red Cross External applicants: two
years recent related nursing experience Internal applicants with
less than two years of experience but post orientation/90-day
probationary period could be considered with Chief Nursing Officer
for level review and approval We will consider licensed military RN
experience Preferred qualifications for the ideal future caregiver
include: Bachelor’s of Science in Nursing (BSN) Physical
Requirements: Requires full range of body motion including handling
and lifting patient, manual and finger dexterity, and eye-hand
coordination. Requires standing and walking for extended periods of
time. Requires corrected vision and hearing to normal range.
Requires working under stressful conditions and irregular hours.
Exposure to communicable diseases and/or body fluids. Medium Work -
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25
pounds of force frequently, and/or greater than negligible up to 10
pounds of force constantly to move objects. Physical Demand
requirements are in excess of those for Light Work. Personal
Protective Equipment: Follows standard precautions using personal
protective equipment as required. Pay Range Minimum hourly: $41.00
Maximum hourly: $41.00 The pay range displayed on this job posting
reflects the anticipated range for new hires. A successful
candidate’s actual compensation will be determined after taking
factors into consideration such as the candidate’s work history,
experience, skill set and education. The pay range displayed does
not include any applicable pay practices (e.g., shift
differentials, overtime, etc.). The pay range does not include the
