Physical Therapist Location: Merrimack, NH Agency: Cynet Health Pay: $2,668 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 4/20/2026 About the Position Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years required License: NH license or compact PT license required Certifications: Wound Care Must-Have: EPIC experience required Unit accepts first time travelers No weekend requirement Local candidates considered within 50 miles Description: Provide physical therapy services to patients in their homes. Perform patient assessments and develop individualized treatment plans. Facilitate lifting and transfers, ensuring safe patient transport to and from treatment. Schedule appointments and handle telephone calls related to patient care. Maintain accurate documentation using the required charting system. Assess patient progress and adjust treatment plans as necessary. Educate patients and families on exercise programs and health management. Ensure compliance with safety protocols and best practices in home health settings. Utilize skills in cardiopulmonary, geriatrics, neurology, oncology, orthopedics, pediatrics, sports medicine, women's health, and wound care. Conduct assessments related to pain, sensory response, and environmental barriers. Monitor vital signs and maintain patient safety throughout therapy sessions. Work collaboratively with interdisciplinary teams to provide comprehensive care. Demonstrate strong time management skills to maximize patient visits and outcomes.
