patient panel with dedicated clinical staff Collaborative
environment with onsite specialty support Clinical Details:
Monday-Friday schedule Outpatient primary care practice Shared call
with manageable volume Provide comprehensive adult primary care
including: Preventive care and wellness visits and Management of
acute and chronic conditions Coordinate care with specialists and
ancillary services Maintain documentation within electronic medical
record system Participate in quality improvement initiatives
