KINGSBURY AS NEEDED - THIS FORM MUST BE FILLED OUT BY THE TRAVELER
AND UPLOADED WITH PROFILE! CARDIAC DRIP EXPERIENCE REQUIRED FOR
THIS NEED. NOTE EXPERIENCE ON PROFILE DEMENTIA
EXPERIENCE/MANAGEMENT REQUIRED NIHSS strongly preferred EPIC
experience This is a small community without public transport.
Traveler candidate must have their own vehicle. This is a firm
requirement. ALL medical/occ health compliance items MUST be
ordered/collected/resulted within 5 days of offer acceptance for
this client. Proofs MUST be uploaded to Trio.
