Job Description:
North Suffolk is looking for a Housing Coordinator to join our
team! The Housing Coordinator works with a dedicated team of staff
working with clients in the areas of engagement, assessment,
treatment planning and in particular, helps persons to secure and
maintain independent housing. This person has expertise in the
housing and subsidy process and is a key player in the goal of
facilitating movement into permanent independent housing.
Salary starts at $48,000 and is based on certifications and
experience. We are currently offering a $3,000 sign on bonus for
this position.
What you'll do:
- Engage people and partner with them to improve their health and
wellness, achieve symptom stabilization and self-management,
develop and/or restore skills impacted by mental illness and live a
self-directed life.
- Assist people with psychiatric and substance use disorders to
identify personal priorities, preferences, strengths, and interests
in order to help them establish goals that support a life in
recovery.
- Provide direct care services in timely and flexible ways.
- Participate in the development and implementation of Safety
Plans and Crisis Prevention and Response Plans in order to help
prevent and mitigate risk factors.
- Work side by side with persons and Team staff to apply for
housing subsidies and decent affordable housing.
- Be an "expert" in the area of housing and serves as a resource,
providing training and consultation for Team staff.
- Facilitate informed decision-making by educating persons about
laws and regulations that affect their housing status and
subsidies.
- Act as liaison with DMH, staff, family members, advocates, and
local, state, and federal housing staff and programs as needed.
What you'll need:
- Education:
- High School Diploma/GED equivalent required.
- Bachelors Degree highly preferred.
- Experience:
- Experience in managing affordable housing and/or housing
support experience required.
- Knowledge of state and federal housing subsidy processes and
DHCD and DND protocols
- Skills:
- Must have Driver's license.
- Must be MAP certified and CPR/1st aide certified within the
first three months of hire (training provided). Competitive &
