Job Description:
A leading critical access hospital located in northern New
Hampshire is now seeking an experienced Primary Care Advanced
Practice Provider to join their award-winning healthcare team.
This full service critical access facility provides care for 20+
communities throughout northern New Hampshire and Vermont. This
community based medical center provides more than 4,000 emergency
visits, 300+ inpatient admissions, and more than 140,000 diagnostic
tests annually. Among many specialties, this location offers 24/7
emergency care, surgery, rehab services, inpatient care, ambulatory
care, and laboratory and imaging services.
Under limited supervision, the Primary Care Advanced Practice
Provider will perform patient assessments, ensure thorough medical
histories, orders and interprets laboratory and diagnostic tests,
develops medical diagnosis, creates specialized treatment plans,
and serves as a patient educator. The Advanced Practice Provider
will work closely with physician and clinical staff, supports the
needs of the outpatient population by offering telehealth or in
person communication, and emphasizes quality patient care.
What's Needed to Succeed:
Advanced Practice Registered Nurse APRN or Physician Assistant
PA-C
Licensed in state of New Hampshier or ability to obtain
Prior working experience is a plus!
