potential. Summary: The Housing Case Manager is responsible for
supporting individuals in obtaining and maintaining housing,
maximizing benefits, working with external agencies, establishing
community relationships, and accessing internal resources as
needed. Schedule: Monday-Friday 9:00am-5:00pm (40 Hours) Essential
Duties & Responsibilities: Develop and maintain positive
relationships with local service agencies, providers, realtors, and
community landlords. Collect tenant rents and follow through on
late rent collections including distribution of notices. Submit
rents to Bridgewell financial services department. Conduct
apartment inspections quarterly and/or as situation may warrant.
Generate correspondence/reports for supervisor and follow-up
letters to tenants as needed. Mediate with Property Manager common
in-house disputes between tenants. Report maintenance needs
reported by the tenant to Bridgewell Facilities Department in a
timely manner. Provide support to individuals with completing HUD
housing applications/education applications and other needs
associated with housing and continuing education documents.
Document housing progress for each tenant using HUD HMIS system and
tier client medical records. Conduct individual and group
informational/educational sessions focused on ensuring housing
stabilization, housing challenges and other issues. Work with
individuals on maximizing benefits and income including
entitlements, work opportunities, and insurance management.
Maintain required documentation according to HUD and all applicable
regulatory standards. Maintain applicable core training
certifications. Other duties as assigned. Required
Education/Experience: Preferred educational background: Associate's
Degree, or working towards degree in related field, or High School
diploma or GED with a minimum of 2 years of successful employment
within a housing and/or benefits maximization focused program.
Experience managing individuals with substance abuse and mental
health conditions. Required Skills/Knowledge: Knowledge of housing
services, challenges, and resources within demographic area
Knowledge of Massachusetts entitlements, housing subsidies,
vocational opportunities, and State insurance programs. Excellent
organizational and problem solving skills Warmth, empathy, and
ability to set limits while maintaining a positive and respectful
attitude. Flexibility, good judgment, organizational skills, and
reliable Proficient in using computer, word processing,
spreadsheets, navigation of internet Bilingual/bicultural preferred
especially in Spanish or Khmer. Physical Demands: The physical

While performing the duties of this job, the employee is frequently
required to climb stairs. The employee must be able to lift and/or
